My Ipod touch 4g has been having a volume problem, when i plug it in to headphones.?

Everytime I plug headphone in to my ipod the volume, goes low. lower than usual. i have went to settings, and made sure the volume is all louder than what it is. i have hit the volume+ button on the side too. I cant quite figure it out. please help!
